Client Waiver and Release of Liability

IMPORTANT: Please read the waiver below carefully, complete all required fields, provide your signature, and submit the form before proceeding.
Services Covered
This waiver applies to wellness services and products offered by Just Retreat Express, including:
- Zero gravity chair massage
- Red light therapy
- Salt foot soaks
- Aromatherapy
- Virtual reality meditation
- Vibration plate therapy
- Infrared sauna therapy
- Salt lounge therapy
- Events and group sessions
- Wellness merchandise
Health and Safety
By participating, you confirm that:
- You are physically and mentally able to participate safely.
- You have disclosed any relevant health conditions, allergies, sensitivities, physical limitations, or pregnancy.
- You will stop participating and notify staff if you experience pain, discomfort, dizziness, or another concerning reaction.
- You have consulted a healthcare provider when appropriate.
- You understand that these services are provided for relaxation and general wellness and are not substitutes for medical or mental health treatment.
Personal Belongings and Electronic Devices
Phones must be silenced and securely stored before participating in a service.
Guests are encouraged to leave unnecessary personal belongings at home or securely stored in their vehicle.
You accept responsibility for securing your belongings and understand that Just Retreat Express is not responsible for lost, stolen, misplaced, or damaged personal items.
Assumption of Risk
You understand that participating in wellness services may involve risks, including discomfort, physical reactions, or injury.
You voluntarily choose to participate and accept responsibility for communicating any discomfort, health concern, or physical limitation to staff.
Waiver and Release of Liability
In consideration for participating in a service or using a product:
- You release and hold harmless Just Retreat Express, its owners, staff, affiliates, and partners from claims for injury, loss, or damage resulting from your participation or use of merchandise, to the extent permitted by applicable law.
- You accept responsibility for your participation and acknowledge that you are participating voluntarily and at your own risk.
Minor Participation
A participant under 18 years of age must have permission from a parent or legal guardian.
The parent or legal guardian must provide their name, relationship to the participant, acknowledgment of permission, and signature before the minor may participate.
